December 2012 Distribution
Various forms (estate, guardianship and conservatorship, mental commitment, child protective and juvenile delinquency, and other family division) were revised by the Michigan Court Forms Committee at its annual fall meetings. Forms Revised Specifically for Use by Pro Se Litigants
Various forms were revised during 2012 through a special work group of the Solutions on Self-Help Task Force. These forms included debt-collection pleadings and postjudgment motions and orders, small claims, landlord-tenant, and setting aside convictions and adjudications. Revised Forms with Specific Effective Dates
The following forms were revised outside the normal revision process because of statutory and court rule changes that were immediately effective or were effective at some point during the past year. They are in order of the most recent effective date.
Effective march 28, 2013, Pursuant to 2012 pa 596
PC 572, Letters of Authority for Personal Representative
These forms were revised to indicate that a debt must be deducted from the value of real property before an estate administration fee is calculated.
Effective January 1, 2013, Pursuant to amended MCR 2.603
MC 07a, Default Request, Affidavit, Entry, and Judgment (Sum Certain)
This form was revised to clarify that a court clerk may enter a default judgment if the requested damages are less than the amount claimed in the original complaint, to reflect payments that may have been made or otherwise credited.
Effective January 1, 2013, pursuant to new training standards and procedures
These forms were revised to comply with revised mediator training requirements.
Effective December 28, 2012, pursuant to 2012 PA 527
These forms were revised to expand the number of adjudications that can be set aside. Instructions are provided with the forms. Additional help is available from Michigan Legal Help.

These forms were revised to accommodate various statutory amendments concerning legally incapacitated and other protected individuals. An explanation of the changes is available here.
Effective September 25, 2012, Pursuant to 2012 PA 304
These forms were revised to reflect a change to MCL 600.4012 that extended the effective period of a writ of garnishment from 91 days to 182 days. The Supreme Court also amended MCR 3.101 to extend the effective period to all periodic garnishments.

JC 17, Order of Disposition (Child Protective Proceedings)
These forms were revised to provide for entry of an ex parte emergency removal order allowing the Department of Human Services to take protective custody of a child and to permit referees to issue these orders in certain instances.
Effective May 22, 2012, Pursuant to 2012 PA 139
DC 100c, Notice to Quit to Recover Possession of Property
DC 100e, Demand for Possession, Termination of Tenancy Due to Unlawful Drug Activity on Premises
These forms were revised to accommodate a statutory amendment that provides for recovery of proceedings after a 7-day notice to quit if a tenant physically injures or threatens to physically injure another person in certain instances.
Effective May 1, 2012, Pursuant to amended mcr 7.109 and mcr 7.121
MC 57, Certificate of Records Transmitted for Appeal and Notice to Parties
CC 80, Order Following Appeal on Application for Concealed Weapon License
MC 57, which was revised last spring to comply with amendments to MCR 7.109(G) (effective May 1, 2012), was further revised to indicate that it can be used in agency appeals. CC 79 and CC 80 were revised to reflect amendments to MCR 7.121 that changed the procedure for concealed weapon license appeals.
