Index of SCAO-Approved Forms for Use in

Mental Health Proceedings

 

 

This set of forms is for treatment of a mentally ill minor or adult. The forms are usually filed in the probate court; however, they may also be filed in the family division of circuit court in ancillary proceedings. See also index for general probate forms which are used in mental health proceedings as appropriate.
